To get the federally approved document you need to bring in:
1. Proof of Citizenship or Lawful Status:
* U.S. Certified Birth Certificate issued by a state or local government (cannot accept hospital-issued birth certificates; cannot accept photocopies)
* U.S. Passport or Passport Card (current, not expired)
* Certificate of Naturalization or Citizenship (N-550, N-560, N-561, N-570)
* U.S. Consular Report of Birth Abroad (FS-240, DS-1350, F-545)
* Foreign Passport with a valid Visa & I-94, valid I-551 stamp, or a valid I-551 statement on the Visa
* Unexpired Permanent Resident Card (I-551)
* Employment Authorization Card (I-688B, I-766) with photo accompanied by documentation approved by Department of Homeland Security (e.g., I-797/I-589) that specifically states valid U.S. lawful status as defined by 6 C.F.R. 37.3

(Needed only if the name on your birth certificate or passport does not match the name on your license, permit, or non-driver ID. If multiple name changes, you may have to bring in all of the name change proofs for each name change in order to link the names together; unless the name change document lists the name that is on your birth cert or passport and also lists the name that is on your current license.)

Absolutely NOT the DMV of old. I had an appointment, was called a few minutes EARLY and flew through the 2 transactions I needed help with. Done in less than 10 minutes! I did mention that I would not have needed the appointment in the first place if the my DMV website worked at all.
